What is not clear to me is that if I install your extension, and when I call it to action, then will the title of the page from which the action call is made get transmitted to me by e-mail or not?

No, but as I said, you can add a custom Subject line for each form if you wanted.
Is mail chimp integration possible?
No , this is not possible.
Does the form data get stored in any table?
No, there is no interaction with the db
Is it possible to obtain the IP address and other related user information?
Yes, this is currently possible
Is it possible to have a 2 column layout?
Not currently. It would depend on what is in the layout, what fields etc ... The current concept if for mobile friendliness.

Hi
A link to the page from where the form was transmitted will be sent in the email. You can also enter a custom Subject line for each instance of the Module.
